Can be manipulated by a reasonably foreseeable method to identify a specific individual; or<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">c. Can be linked with other available information to identify a specific client.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">4. We adopted this policy to be consistent with standards issued by the Department of Housing and Urban Development (HUD) for Homeless Management Information Systems. See 69 Federal Register 45888 (July 30, 2004).<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">5. This policy informs our clients, our staff, and others how we process personal information. We follow the policy and practices described in this privacy notice.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">6. We may amend our policy or practices at any time. Amendments may affect PII that we obtained before the effective date of the amendment.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">7. We give a written copy of this privacy notice to any individual who asks for it.<\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-weight: 400;\">8. We maintain a copy of this notice on our website at <\/span><a href=\"https:\/\/restorenyc.org\/zh\/\" style=\"color: #000000;\"><span style=\"font-weight: 400;\">www.restorenyc.org<\/span><\/a><span style=\"font-weight: 400;\"> (navigate to \u201cWhat We Do\u201d and select the Privacy Notice within the \u201cHousing Services\u201d section).\u00a0<\/span><\/span><\/p>\n<p><span style=\"text-decoration: underline;\"><span style=\"color: #000000;\"><b>How and Why We Collect PII.<\/b><\/span><\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">1. We collect PII only when appropriate to provide services or for another specific purpose of our organization or when required by law. We may collect information for these purposes:\u00a0<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">a. To provide individual case management;\u00a0<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">b. To produce aggregate-level reports regarding use of services;<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">c. To track individual project-level outcomes;<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">d. To identify unfilled service needs and plan for the provision of new services;<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">e. To conduct research for consulting and\/or educational purposes; and<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">f. To accomplish any and all other purposes deemed appropriate by the HUD Continuum of Care (CoC).<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">2. We only use lawful and fair means to collect PII.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">3. We normally collect with the knowledge or consent of our clients. If you seek our assistance and provide us with PII, we assume that you consent to the collection of information described in this policy.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">4. We share this data with the NYC Department of Social Services (DSS), Federal Homeless Policy and Reporting unit (FHPR) a\/k\/a\/ the \u201cHUD CoC unit\u201d: the agency appointed by the CoC to manage all PII we record about our clients. This agency is required to maintain the confidentiality of the data.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">5. We post a sign at our intake desk or other location explaining why we ask for PII that says: <\/span><\/p>\n<p><span style=\"color: #000000;\"><i><span style=\"font-weight: 400;\">\u201cSome funders require us to collect personal information for reasons outlined in our Privacy Policy. You can request a copy of the Privacy Policy and\/or your information that has been collected at any time.\u00a0<\/span><\/i><\/span><\/p>\n<p><span style=\"color: #000000;\"><i><span style=\"font-weight: 400;\">The personal information we collect is stored in a system called Bonterra Apricot. Personal information about individuals enrolled in our Housing services is also shared with a system called the Homeless Management Information System (HMIS). The collected information helps us run our programs, improve services, and better understand your needs. We only collect information that we consider to be appropriate or is required by our <\/span><\/i><\/span><span style=\"color: #000000;\"><i><span style=\"font-weight: 400;\">funders.\u00a0<\/span><\/i><\/span><\/p>\n<p><span style=\"color: #000000;\"><i><span style=\"font-weight: 400;\">Please speak to the staff member you are working with if you have any questions or concerns about this notice.\u201d<\/span><\/i><\/span><\/p>\n<p><span style=\"text-decoration: underline;\"><span style=\"color: #000000;\"><b><\/b><\/span><\/span><\/p>\n<p><span style=\"text-decoration: underline;\"><span style=\"color: #000000;\"><b>How We Use and Disclose PII.<\/b><\/span><\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">1. We use or disclose PII for activities described in this part of the notice. We may or may not make any of these uses or disclosures of your PII. We assume that you consent to the use or disclosure of your PII for the purposes described below and for other uses and disclosures that we determine to be compatible with these uses or disclosures:<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">a. To provide or coordinate services to individuals;\u00a0<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">b. When required by law to the extent that use or disclosure complies with and is limited to the requirements of the law;<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">f. To avert a serious threat to health or safety if:<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">i. We believe that the use or disclosure is necessary to prevent or lessen a serious and imminent threat to the health or safety of an individual or the public; and\u00a0<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">ii. The use or disclosure is made to a person reasonably able to prevent or lessen the threat, including the target of the threat.<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">g. To report about an individual we reasonably believe to be a victim of abuse, neglect or domestic violence to a governmental authority (including a social service or protective services agency) authorized by law to receive reports of abuse, neglect or domestic violence in any of the following three circumstances:<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">i. Where the disclosure is required by law and the disclosure complies with and is limited to the requirements of the law;<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">ii. If the individual agrees to the disclosure; or<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">iii. To the extent that the disclosure is expressly authorized by statute or regulation and either of the following are applicable:<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">1. We believe the disclosure is necessary to prevent serious harm to the individual or other potential victims; or<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">2. If the individual is unable to agree because of incapacity, a law enforcement or other public official authorized to receive the report represents that the PII for which disclosure is sought is not intended to be used against the individual and that an immediate enforcement activity that depends upon the disclosure would be materially and adversely affected by waiting until the individual is able to agree to the disclosure.<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">iv. When we make a permitted disclosure about a victim of abuse neglect or domestic violence, we will promptly inform the individual who is the victim that a disclosure has been or will be made, except if:<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">1. We, in the exercise of professional judgment, believe informing the individual would place the individual at risk of serious harm; or<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">2. We would be informing a personal representative (such as a family member or friend), and we reasonably believe the personal representative is responsible for the abuse, neglect or other injury, and that informing the personal representative would not be in the best interests of the individual as we determine in the exercise of our professional judgment.<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">h. To a law enforcement official for a law enforcement purpose (if consistent with applicable law and standards of ethical conduct) under any of these circumstances:<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">i. In response to a lawful court order, court-ordered warrant, subpoena or summons issued by a judicial officer, or a grand jury subpoena;<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">ii. If the law enforcement official makes a written request for PII that:<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">1. Is signed by a supervisory official of the law enforcement agency seeking the PII;<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">2. States that the information is relevant and material to a legitimate law enforcement investigation;<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">3. Identifies the PII sought;\u00a0<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">4. Is specific and limited in scope to the extent reasonably practicable in light of the purpose for which the information is sought; and\u00a0<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">5. States that de-identified information could not be used to accomplish the purpose of the disclosure.<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">iii. If we believe in good faith that the PII constitutes evidence of criminal conduct that occurred on our premises;<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">iv. In response to an oral request for the purpose of identifying or locating a suspect, fugitive, material witness or missing person and the PII disclosed consists only of name, address, date of birth, place of birth, social security number and distinguishing physical characteristics; or if:<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">1. The official is an authorized federal official seeking PII for the provision of protective services to the President or other persons authorized by 18 U.S.C. 3056, or to foreign heads of state or other persons authorized by 22 U.S.C. 2709(a)(3), or for the conduct of investigations authorized by 18 U.S.C. 871 and 879 (threats against the President and others); and<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">2. The information requested is specific and limited in scope to the extent reasonably practicable in light of the purpose for which the information is sought.<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">i. To comply with government reporting obligations for the Homeless Management Information System (HMIS) and for oversight of compliance with HMIS requirements.<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">j.To third parties for the following purposes:<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">i. To permit other systems of care to conduct data matches (i.e., to determine if you are also utilizing services from such other systems of care); and<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">ii. To permit third-party research firms and\/or evaluators to perform research and evaluation services in connection with the programs administered by the CoC and the other agencies;<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">1. Provided that before PII is disclosed under this subsection, the third party that will receive such PII and use it as permitted above must first execute a Data Use &amp; Disclosure Agreement requiring such third party to comply with all applicable laws and regulations, including the privacy standards and disclosure provisions contained in the Department of Housing and Urban Development Homeless Management Information Systems; Data and Technical Standards Final Notice (see 69 Federal Register 45888 (July 30, 2004)), which such standards and provisions are reflected herein.<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">2. Before we make any use or disclosure of your PII that is not described here, we seek your consent first.<\/span><\/p>\n<p><span style=\"text-decoration: underline;\"><span style=\"color: #000000;\"><b>How to Inspect and Correct PII.<\/b><\/span><\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">1. You may inspect and have a copy of your PII that we maintain. We will offer to explain any information that you may not understand.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">2. We will consider a request from you for correction of inaccurate or incomplete PII that we maintain about you. If we agree that the information is inaccurate or incomplete, we may delete it or we may choose to mark it as inaccurate or incomplete and to supplement it with additional information.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">3. We may deny your request for inspection or copying of PII if:<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">a. The information was compiled in reasonable anticipation of litigation or comparable proceedings;<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">b. The information is about another individual (other than a health care provider or homeless provider);<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">c. The information was obtained under a promise of confidentiality (other than a promise from a health care provider or homeless provider) and if the disclosure would reveal the source of the information; or<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">d. Disclosure of the information would be reasonably likely to endanger the life or physical safety of any individual.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">4. If we deny a request for access or correction, we will explain the reason for the denial. We will also include, as part of the PII that we maintain, documentation of the request and the reason for the denial.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">5. We may reject repeated or harassing requests for access to or correction of PII.<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"text-decoration: underline;\"><span style=\"color: #000000;\"><b>Data Retention.<\/b><\/span><\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">1. We collect only PII that is relevant to the purposes for which we plan to use it. El funcionario es un funcionario federal autorizado que busca PII para la prestaci\u00f3n de servicios de protecci\u00f3n al Presidente u otras personas autorizadas por el T\u00edtulo 18 del C\u00f3digo de los Estados Unidos 3056, o a jefes de estado extranjeros u otras personas autorizadas por el T\u00edtulo 22 del C\u00f3digo de los Estados Unidos 2709(a)(3), o para la realizaci\u00f3n de investigaciones autorizadas por el T\u00edtulo 18 del C\u00f3digo de los Estados Unidos 871 y 879 (amenazas contra el Presidente y otros); y<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">2. La informaci\u00f3n solicitada es espec\u00edfica y limitada en su alcance en la medida razonablemente posible a la luz del prop\u00f3sito para el cual se busca la informaci\u00f3n.<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">i. Para cumplir con las obligaciones de informes del gobierno para el Sistema de Informaci\u00f3n de Gesti\u00f3n de Personas sin Hogar (HMIS, por sus siglos en ingl\u00e9s) y para supervisar el cumplimiento de los requisitos del HMIS.<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">j. A terceros para los siguientes fines:<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">i. Para permitir que otros sistemas de atenci\u00f3n realicen comparaciones de datos (es decir, para determinar si tambi\u00e9n est\u00e1 utilizando servicios de esos otros sistemas de atenci\u00f3n); y<\/span><\/p>\n<p style=\"padding-left: 80px;\"><span style=\"font-weight: 400; color: #000000;\">ii. Para permitir que empresas de investigaci\u00f3n y\/o evaluadores de terceros realicen servicios de investigaci\u00f3n y evaluaci\u00f3n en relaci\u00f3n con los programas administrados por el CoC y otras agencias;<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">1. Siempre que antes de que se divulgue PII seg\u00fan esta subsecci\u00f3n, el tercero que recibir\u00e1 dicha PII y la utilizar\u00e1 seg\u00fan lo permitido anteriormente debe primero firmar un Acuerdo de uso y divulgaci\u00f3n de datos que requiere que dicho tercero cumpla con todas las leyes y regulaciones aplicables, incluidas las normas de privacidad y las disposiciones de divulgaci\u00f3n contenidas en el Aviso final de normas t\u00e9cnicas y de datos de los sistemas de informaci\u00f3n de gesti\u00f3n de personas sin hogar del Departamento de Vivienda y Desarrollo Urbano (consulte el Registro Federal 69 45888 del 30 de julio de 2004), cuyos est\u00e1ndares y disposiciones se reflejan en el presente documento.<\/span><\/p>\n<p style=\"padding-left: 120px;\"><span style=\"font-weight: 400; color: #000000;\">2. Puede inspeccionar y tener una copia de su PII que mantenemos. Le ofreceremos una explicaci\u00f3n sobre cualquier informaci\u00f3n que no entienda. El PII que mantenemos puede ser disponible \u00fanicamente en ingl\u00e9s, y no podemos traduc\u00edrselo. Consideraremos su solicitud de correcci\u00f3n de informaci\u00f3n de identificaci\u00f3n personal inexacta o incompleta que tengamos sobre usted. Si aceptamos que la informaci\u00f3n es inexacta o incompleta, podemos eliminarla o podemos elegir marcarla como inexacta o incompleta y complementarla con informaci\u00f3n adicional.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">3. Podemos rechazar su solicitud de informaci\u00f3n de inspecci\u00f3n o copia de PII si:<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">a. La informaci\u00f3n fue compilada con anticipaci\u00f3n razonable a un litigio o procedimientos comparables;<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">b. La informaci\u00f3n es sobre otra persona (que no sea un proveedor de atenci\u00f3n m\u00e9dica o un proveedor de personas sin hogar);<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">c. La informaci\u00f3n fue obtenida bajo una promesa de confidencialidad (que no sea una promesa de un proveedor de atenci\u00f3n m\u00e9dica o un proveedor de personas sin hogar) y si la divulgaci\u00f3n revelar\u00eda la fuente de la informaci\u00f3n; o<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">d. La divulgaci\u00f3n de la informaci\u00f3n ser\u00eda razonablemente probable que pusiera en peligro la vida o la seguridad f\u00edsica de cualquier persona.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">4. Si denegamos una solicitud de acceso o correcci\u00f3n, explicaremos el motivo de la denegaci\u00f3n. Recopilamos solo PII que sea relevante para los fines para los que planeamos utilizarla. En la medida necesaria, buscamos mantener solo informaci\u00f3n de identificaci\u00f3n personal que sea precisa, completa y oportuna.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">2. Eliminaremos la informaci\u00f3n de identificaci\u00f3n personal que no est\u00e9 en uso siete a\u00f1os despu\u00e9s de que la informaci\u00f3n haya sido creada o modificada por \u00faltima vez. Como alternativa a la eliminaci\u00f3n, podemos optar por eliminar los identificadores de la informaci\u00f3n de identificaci\u00f3n personal.<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">3. color: #000000;\">4. \u6211\u4eec\u91c7\u7528\u8fd9\u4e9b\u653f\u7b56\u662f\u4e3a\u4e86\u4e0eDepartment of Housing and Urban Development (HUD)\u4f4f\u623f\u548c\u57ce\u5e02\u53d1\u5c55\u90e8\u53d1\u5e03\u7684for Homeless Management Information Systems\uff08\u65e0\u4f4f\u623f\u8005\u7ba1\u7406\u4fe1\u606f\u7cfb\u7edf\uff09\u4e2d\u6240\u4f7f\u7528\u7684\u6807\u51c6\u4fdd\u6301\u4e00\u81f4\u3002\u53c2\u89c1 69 Federal Register 45888\uff082004 \u5e74 7 \u6708 30 \u65e5\uff09\u3002<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">5. \u8fd9\u4e9b\u653f\u7b56\u544a\u77e5\u6211\u4eec\u7684\u5ba2\u6237\u3001\u5458\u5de5\u548c\u5176\u4ed6\u4eba\u6211\u4eec\u5982\u4f55\u5904\u7406\u4e2a\u4eba\u4fe1\u606f\u3002\u6211\u4eec\u9075\u5faa\u672c\u9690\u79c1\u653f\u7b56\u4e2d\u6240\u8ff0\u7684\u653f\u7b56\u548c\u60ef\u4f8b\u3002<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">6. \u6211\u4eec\u53ef\u80fd\u968f\u65f6\u4fee\u6539\u6211\u4eec\u7684\u653f\u7b56\u6216\u60ef\u4f8b\u3002\u4fee\u8ba2\u53ef\u80fd\u4f1a\u5f71\u54cd\u6211\u4eec\u5728\u4fee\u8ba2\u751f\u6548\u65e5\u671f\u4e4b\u524d\u83b7\u5f97\u7684 PII\u3002<\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">7. \u6211\u4eec\u4f1a\u5411\u4efb\u4f55\u7d22\u8981\u672c\u9690\u79c1\u653f\u7b56\u7684\u4e2a\u4eba\u63d0\u4f9b\u4e00\u4efd\u4e66\u9762\u526f\u672c\u3002<\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-weight: 400;\">8. \u6211\u4eec\u5728\u6211\u4eec\u7684\u7f51\u7ad9\u4e0a\u4fdd\u7559\u4e00\u4efd\u8fd9\u4e9b\u653f\u7b56\u7684\u526f\u672c <\/span><a href=\"https:\/\/restorenyc.org\/zh\/\" style=\"color: #000000;\"><span style=\"font-weight: 400;\">www.restorenyc.org<\/span><\/a><span style=\"font-weight: 400;\"> (\u70b9\u51fb\u201c\u6211\u4eec\u6240\u505a\u7684\u201d \u5e76\u9009\u62e9\u5728\u201cHousing Services\u201d\u4e2d\u7684Privacy Notice).\u00a0<\/span><\/span><\/p>\n<p><span style=\"color: #000000;\">\u00a0<\/span><\/p>\n<p><span style=\"text-decoration: underline; color: #000000;\"><b>\u6211\u4eec\u6536\u96c6 PII \u7684\u65b9\u5f0f\u548c\u539f\u56e0<\/b><\/span><\/p>\n<p><span style=\"font-weight: 400; color: #000000;\">1. \u6211\u4eec\u4ec5\u5728\u9002\u5f53\u7684\u65f6\u5019\u6536\u96c6 PII\uff0c\u4ee5\u7528\u4e8e\u63d0\u4f9b\u670d\u52a1\u6216\u7528\u4e8e\u6211\u4eec\u7ec4\u7ec7\u7684\u5176\u4ed6\u7279\u5b9a\u76ee\u7684\uff0c\u6216\u5728\u6cd5\u5f8b\u8981\u6c42\u65f6\u5fc5\u987b\u6536\u96c6\u7684\u65f6\u5019\u3002\u6211\u4eec\u53ef\u80fd\u51fa\u4e8e\u4ee5\u4e0b\u76ee\u7684\u6536\u96c6\u4fe1\u606f:\u00a0<\/span><\/p>\n<p style=\"padding-left: 40px;\"><span style=\"font-weight: 400; color: #000000;\">4. \u6211\u4eec\u4e0eNYC Department of Social Services (DSS), Federal Homeless Policy and Reporting unit (FHPR)\u53c8\u540dHUD CoC unit\u5171\u4eab\u8fd9\u4e9b\u6570\u636e\u3002\u8be5\u673a\u6784\u5fc5\u987b\u5bf9\u8fd9\u4e9b\u6570\u636e\u4fdd\u5bc6\u3002<\/span><\/p>\n<p><span style=\"font-weight: 400;
